Immunization and Health Records
IMMUNIZATION RECORDS
Connecticut State Law, Public Act 89-90 specifically mandates all matriculated post-secondary students must be properly immunized. In accordance with this state law, all matriculated students born on or after 1/1/57, who are enrolled in the School of Social Work graduate program, must show evidence of immunity to measles and rubella. In order to meet State law requirements, all students must have evidence of 2 Measles Vaccinations and 1 Rubella Vaccination. All vaccinations must be after the first birthday and 1/1/69. The second measles vaccination must be after 1/1/80.
Students can show proof of immunity by titer or vaccination history/record. All students must complete an immunization form, regardless of whether they were born before or after 1/1/57. Students who do not submit a form that meets all requirements, may be administratively withdrawn from classes, since all matriculated students must be properly immunized to attend classes at the School of Social Work. Forms may be obtained from the Admissions Office, Room #l04A, and must be returned prior to the start of fall classes. Students requiring an additional MMR (Measles/Mumps/Rubella) vaccine may obtain the MMR vaccination at a cost of $10 from West Hartford Public Health Services (located in West Hartford Town Hall), 50 South Main Street, 4th Floor, Room 426, West Hartford. The immunization clinic is open only on Wednesday afternoons from 1:30 to 4:30 p.m. by appointment only; please call 561-7900 for more information.

ADDITIONAL STUDENT HEALTH RECORDS REQUIREMENT
Additional student health records information may be required if students are employed or conducting a field placement in a health care agency, hospital or clinic. Students may be required to complete an additional health verification form showing evidence of measles, rubella, non-reactive PPD (not more than 6 months old) or documentation of treatment and resolution of active TB episode, or documentation of negative chest X-ray after a positive PPD, documentation of current varicella (chickenpox) or titer or verbal history, documentation of acceptance or declination of hepatitis B immunization.
